74781c0888 exec/cpu: Extract page-protection definitions to page-protection.h 
Extract page-protection definitions from "exec/cpu-all.h"
to "exec/page-protection.h".

The list of files requiring the new header was generated
using:

$ git grep -wE \
  'PAGE_(READ|WRITE|EXEC|RWX|VALID|ANON|RESERVED|TARGET_.|PASSTHROUGH)'

a43972e176 target/mips: Replace [g_]assert(0) -> g_assert_not_reached() 
In order to avoid warnings such commit c0a6665c3c ("target/i386:
Remove compilation errors when -Werror=maybe-uninitialized"),
replace all assert(0) and g_assert(0) by g_assert_not_reached().

Remove any code following g_assert_not_reached().

See previous commit for rationale.

a844873512 mips: Remove support for trap and emulate KVM 
This support was limited to the Malta board, drop it.
I do not have a machine that can run VZ KVM, so I am assuming
that it works for -M malta as well.

137f4d87c6 target/mips: Move physical addressing code to sysemu/physaddr.c 
Declare get_physical_address() with local scope and move it along
with mips_cpu_get_phys_page_debug() to sysemu/physaddr.c new file.
