- Directly calculate the Qt scale factor, no longer rely on the

Xft.dpi X resource (bsc#1199020)
- Support YAST_MON_WIDTH_MM=xx at installation boot prompt
- Support YaST2.call script debugging with FAKE_MON_WIDTH_MM=xx
- Support up to 392 dpi (up to scale factor 4.0)
