* Fix a possible deadlock in voice callbacks when the
    application has aggressive mutex locking
  * Minor fixes for implicit conversion warnings on MSVC
  * Wine/Win32:
  * Fix a crash when GetDefaultAudioEndpoint fails
  * Exit cleanly when opened audio devices disconnect
  * Properly list all audio devices, rather than just the default
