* Added typing to the default script mako templates.
  * Added support in autogenerate for NULLS NOT DISTINCT in
  * the PostgreSQL dialect.
  * Fixed format string logged when running a post write hook
  * Added parameters if_exists and if_not_exists for index
    operations.
